The fitouts that fail share common characteristics that are recognisable to anyone who has managed commercial construction: late changes that were not communicated to the certifier, trades that did not complete their sign-off documentation, and compliance requirements that were identified during design but not followed through during construction. These are not exotic failures. They are process failures that stem from predictable gaps in coordination and documentation.
Understanding what causes certification failures helps tenants and their fitout teams avoid them. The patterns are consistent enough that most failures can be prevented by managing the known risk points rather than hoping they will not apply to a particular project.
Why Certification Failures Happen More Often Than They Should
Certification is the process through which the completed fitout is assessed against the approved plans and the relevant building codes. In NSW, this assessment is conducted by a private certifier or the local council, and the outcome is either an occupation certificate that allows the tenant to occupy the space or a list of deficiencies that must be rectified before the certificate is issued.

Fire Compliance Failures: The Most Common Category

Fire-stopping at service penetrations through rated walls is the single most frequent failure point. When a fitout creates rooms that did not exist in the original layout, those rooms need conditioned air supply and a return air path. If the HVAC modifications were not completed, were not balanced, or were not tested before the certification inspection, the certifier may identify the deficiency.

Meeting rooms, kitchens, and bathrooms all have minimum ventilation requirements under the Building Code of Australia, and these requirements are based on the room’s use and occupancy, not just its size. A meeting room designed for twelve people has a different ventilation requirement from one designed for four, and if the HVAC system has not been configured to deliver the required rate, the certifier will note the shortfall.
Building a fitout in the correct order ensures that HVAC modifications are completed and commissioned before the ceiling goes in and before the certifier inspects. Where the construction sequence compresses and the ceiling is closed before the HVAC is tested, problems are harder to identify and more expensive to fix.
Electrical Certification Gaps
Electrical compliance in office fitouts is governed by AS/NZS 3000 (the Wiring Rules) and requires all electrical work to be completed by a licensed electrician and tested before the occupation certificate is sought. Common electrical certification gaps include incomplete testing and tagging of circuits, missing certificates of compliance from the electrician, emergency and exit lighting that has not been commissioned or tested, and switchboard labelling that does not reflect the as-built circuit configuration.
Emergency and exit lighting is a particularly frequent failure point. New partitions and enclosed rooms change the egress paths through the tenancy, which changes the locations where exit signs and emergency lighting are required. If the lighting design was based on the original open-plan layout and was not updated to reflect the as-built partition configuration, the certifier will identify missing or incorrectly located exit signs as a deficiency.
The electrical certificate of compliance is a documentation requirement that is sometimes overlooked in the rush to complete the fitout. The certifier cannot issue the occupation certificate without it, and the electrician cannot issue it until all testing is complete. Ensuring that the electrician is scheduled to complete testing and issue the certificate before the certification inspection avoids this easily preventable delay.
Accessibility and Egress Non-Compliance
Accessibility requirements under the Disability Discrimination Act and the Building Code of Australia apply to commercial office fitouts and are assessed as part of the certification process. Common failure points include door widths that do not meet the minimum clearance for wheelchair access, door handles that are not lever-type as required for accessible routes, and sanitary facilities that do not comply with the accessible design requirements where they are required.
Egress non-compliance is typically related to travel distances and exit widths. When a fitout adds partitions that create corridors and enclosed rooms, the travel distance from the most remote point of the tenancy to the nearest exit needs to be assessed against the code limits. If the new layout creates a travel distance that exceeds the allowable maximum, the certifier will require the layout to be modified or additional exit provisions to be made.
Fitouts that appear simple but carry hidden complexity often fall into this category. A straightforward partitioning exercise that creates six new offices and a corridor can trigger egress, accessibility, fire, and HVAC requirements that were not part of the original scope estimate, and if any of these are missed, the certification inspection will find them.
How Compressed Timelines Contribute to Failure
The majority of certification failures share a common root cause: the construction programme was compressed, and the compliance steps that should have happened during the build were deferred to the end. Fire-stopping that should have been inspected before linings went on was not checked. HVAC balancing that should have been completed before the ceiling was closed was skipped. Documentation that should have been compiled throughout the project was assembled in haste on the final day.
Late design changes are a frequent contributor to timeline compression. A change to the partition layout three weeks before completion triggers a cascade of modifications to the fire strategy, HVAC layout, electrical distribution, and exit provisions. If those modifications are not captured in the documentation and not inspected before the certifier arrives, the certification fails not because the fitout is wrong but because the documentation does not reflect what was built.
The solution is to treat compliance as a progressive process rather than a final checkpoint. Inspections at key stages during construction, particularly before walls are closed and before ceilings go in, catch deficiencies when they are easy and cheap to fix rather than at the end when they delay occupation.
Preventing Certification Failure Through Process
The fitouts that pass certification consistently are not the ones with the simplest scope. They are the ones with the best process. That process includes clear documentation of the approved design, progressive inspections during construction, trade sign-off at each stage, and compilation of compliance documentation throughout the build rather than at the end.
A pre-certification review, conducted by the fitout team one to two weeks before the certifier’s inspection, is one of the most effective preventive measures. This review walks through the tenancy with a checklist that mirrors what the certifier will assess: fire-stopping at every penetration, sprinkler and detector coverage in every room, HVAC supply and return in every enclosed space, emergency and exit lighting on every egress path, and accessibility compliance at every required point.
